Former Dowelltown Insurance Provider Faces Fine, Revocation

A former Dowelltown insurance producer has surrendered his insurance license for violating several insurance laws.

An investigation by the State Department of Commerce and Insurance found that Orvil E. Hendrix forged another person’s name to a document related to an insurance transaction and intentionally misrepresented the terms of an insurance contract.

Hendrix has agreed to pay a civil penalty in the amount of 5-thousand dollars.
