A two-vehicle crash in Cookeville injured an adult and five juveniles Monday.
The crash happened on Shag Rag Road at 3:25 p.m.
Cookeville Police said a westbound Dodge Journey collided head-on with an eastbound Dodge Dakota.
The 17-year-old driver and 17-year-old passenger of the Dodge Dokota received treatment at Cookeville Regional Medical Center for unknown injuries.
Police officials identified the driver of the Dodge Journey as 25-year-old Alisha Barlow. A crash report said three juveniles occupied the vehicle, including a 15, 5, and 4-year-old.
The report said a medical helicopter airlifted Barlow and the two oldest children to a hospital for treatment to unknown injuries. The 4-year-old received treatment at CRMC.
The crash remains under investigation.
